The Polk County Health Resource Navigator will enhance the health and wellness system of care for Polk County by helping residents navigate our health and behavioral health systems (primary care, mental health, substance abuse and social services). This position will assist residents of Polk County, with special focus on the uninsured and underserved, who need assistance with their health, mental health, substance abuse, and social needs. This is a grant position and is contingent upon continued funding.
Graduation from a four year college or university with a major in social work or health education and one year of experience in a service organization, preferably health related, in a case management, social worker or clinical role; or an equivalent combination of education and experience.
