Health Check provides free health insurance to children, ages birth to 21, whose families qualify.
NC Health Choice for Children offers free or low-cost health insurance to uninsured children, ages birth to 19, whose families’ gross monthly incomes are at or below 200 percent of the federal poverty level. For a family of four, that amount is $3,142 during 2004-05. For a family whose income is between 150-200% FPL, the NC Health Choice enrollment fee is $50/year per child, but no more than $100 per family. Medical co-pays are $5; generic prescriptions, a little more.
Health Check and NC Health Choice have comprehensive benefits, including dental coverage. The application form and detailed information for both programs may be found at www.dhhs.state.nc.us/dma .
Completed applications should be sent to your county’s Department of Social Services, along with the requested income documents. Applications are reviewed first for Health Check (Medicaid) eligibility. If family income is above the Medicaid eligibility level, but under 200 percent of federal poverty level, the child may be enrolled in NC Health Choice. Parents will be notified if an enrollment fee is due. Health Choice is administered by Blue Cross-Blue Shield of North Carolina and has the same benefits as the State Employees health insurance.
